Copyl Care Management

The individual care process, from first interview to outcome review, with the evidence and the decision of a named person at every step.

Copyl Care Management follows an individual's care from the first interview to the review of the outcome. The interview is structured around IBIC, the assessment draft links every statement to its source, decisions are signed with their reasoning, and the provider acknowledges the handover. Agents propose. A named person decides. Copyl keeps the record.

Copyl Care Management

Built for the people who run the care process

Three roles work in the same process. Each sees what they need to act and nothing more, and all of them see the same record of what was proposed and decided.

Municipal care managers

Assess with the sources in front of you

The interview, the assessment draft and the decision stay in one place, so reasoning can be read back later by anyone who is allowed to see it.

  • Conduct the interview in an IBIC-structured form
  • Review an assessment draft where each statement links to its source
  • Sign the decision with your own reasoning

Home-care providers and unit managers

Receive a plan you can staff

The handover is acknowledged, and the plan arrives as time windows, durations and staff eligibility instead of a free-text note.

  • Acknowledge the handover before the plan starts
  • See time windows, duration and who is eligible for each visit
  • Replan visits when an absence changes who is available

Nurses

Observations and requests that reach the right person

What staff notice in the home, and what a nurse asks for, is tied to the person and the plan, so it can feed the next review.

  • Register field observations against the person's plan
  • Send a nurse request that is routed to the responsible person
  • Follow what happened to the request and why

Six steps, one record

Every step follows the same pattern: Copyl gathers what is known, an agent proposes, a named person decides and the evidence stays attached.

An interview that follows the person's needs.

The care manager conducts the interview in a structure built on IBIC, so the person's needs, goals and wishes are captured the same way each time and can be compared over time.

  • Questions are organised around the person's needs and goals
  • Answers are stored with who gave them and when
  • The care manager can add what the form does not ask for

An assessment draft with its sources.

An agent drafts the assessment from the interview and the documents the care manager is allowed to use. Each statement links to the passage it came from. The draft is a proposal, and the care manager edits, accepts or rejects it.

  • Every statement in the draft links to its source
  • Unsupported statements are flagged instead of being smoothed over
  • Nothing becomes part of the assessment until the care manager accepts it

Signed decisions and an acknowledged handover.

The care manager signs the decision and writes down the reasoning. Copyl does not make clinical or entitlement decisions. The provider then acknowledges the handover, so it is clear who has received what.

  • The decision carries its reasoning and the name of the person who signed it
  • The handover to the provider is acknowledged, not just sent
  • Changes after signing create a new, visible version

A plan the provider can run in Home Care.

The decision becomes a structured implementation plan in Home Care: which visits, in which time windows, for how long and which staff are eligible. The provider's unit manager plans from it directly.

  • Time windows and duration per visit, not a free-text note
  • Staff eligibility stated up front
  • The plan stays linked to the decision it came from

Staffing that knows what the care needs.

Roster plans staff with the care requirements in view. When an absence is registered, Copyl shows which planned visits are affected and an agent proposes a replan. The planner decides.

  • Staff are matched against the requirements in the plan
  • An absence shows which visits it affects
  • A replan is a proposal until the planner approves it

Observations, requests and a review that closes the loop.

Staff register field observations and nurses send requests. At the outcome review, Copyl gathers what has happened since the decision and, where the need seems to have changed, proposes a reassessment. The care manager decides whether to start one.

  • Observations and nurse requests are tied to the person and the plan
  • The outcome review collects what has happened since the decision
  • A reassessment is proposed, never started automatically

How it works: from event to evidence

Event

Something happens: an observation, a nurse request, an absence, a review date.

Signal

Copyl connects related events to the person and the plan they concern.

Proposal

An agent proposes the next step, with its reasoning and the sources it used.

Human decision

A named person approves, changes or rejects the proposal.

Evidence

Proposal, decision and outcome are written to a record that can only be added to.

Two situations, step by step

Both follow the same flow, and in both a person decides.

A changed need leads to a reassessment

  1. 1. Event

Staff register several observations that the person now needs more help in the morning, and a nurse sends a request about the same thing.

  2. 2. Signal

Copyl connects the observations and the request to the person's plan and shows that they point the same way.

  3. 3. Proposal

An agent proposes a reassessment and lists the observations behind it, each linked to its source.

  4. 4. Human decision

The care manager starts the reassessment, asks for more information or decides that nothing should change yet.

  5. 5. Evidence

Observations, proposal and decision are in the record, so the next assessment starts from what was actually seen.

A sick-leave absence affects planned visits

  1. 1. Event

A member of staff reports sick for the coming days.

  2. 2. Signal

Roster shows which planned visits the absence affects and which of those have care requirements on who can perform them.

  3. 3. Proposal

An agent proposes a replan that respects time windows and staff eligibility, and lists any visit it cannot cover.

  4. 4. Human decision

The planner approves the replan, adjusts it or handles the uncovered visits another way.

  5. 5. Evidence

The absence, the proposal and the planner's decision are in the record, with who decided and when.

People decide. Copyl keeps the record.

Agents propose and a named person approves. Copyl does not make clinical or entitlement decisions, and it works alongside the case and journal systems the municipality already uses. Every proposal, decision and outcome is written to a record that can only be added to. Data is hosted in the EU.

Questions from care organisations

Does Copyl replace our journal or case system?

No. Copyl Care Management supports the individual care process and keeps its own record of proposals and decisions. The municipality's journal and case systems stay where they are.

Who makes the decisions?

People do. Agents propose, and a named person approves, changes or rejects. Copyl does not make clinical or entitlement decisions.

How do we know where a statement in the draft comes from?

Each statement in an assessment draft links to the passage it was based on. A statement without support is flagged for the care manager.

Where is our data hosted?

In the EU.

Works with the systems you already have

Copyl does not replace the systems you already run. Your maintenance, finance, inventory, HR and sensor systems connect to Copyl through its integration platform — governed, permission-checked and logged.

You get the apps and the AI agents on top of what you already have. Your existing systems become data sources for the agents.

Copyl Care Management works alongside the municipality's existing case and journal systems and does not replace them.
